If you are enrolled in the CDHP, please look in the left navigation for the separate 2017 medical booklet supplied by BCBS. The booklet is part of your summary plan description (SPD) and is incorporated into the SPD by reference as if it were included herein.
BCBS CDHP and PPO medical booklets for 2018 will be posted as soon as they are available.
